Reminder: Updated Allergy & Medical Form Required

As part of our ongoing commitment to student health and safety, we require all parents and guardians to resubmit the updated Allergy & Medical Information Form by Friday, 15th August 2026.

This is a new version of the form and replaces all previous submissions.

Where to get the form:

• Collect a printed copy from the school office (open Monday–Friday, 8:00 AM – 4:30 PM)
• Download the digital version from the EduFlow parent portal

Completed forms should be returned to your child's class teacher or dropped off at the school office.

If your child has no known allergies or medical conditions, you are still required to complete and return the form. This ensures our records are accurate and up to date.
